At Stony BrookMedicine, a TH Staff Associate/SupervisingCase Manager is a valuable member of our team, who provides clinicalservices to our patient population.

Qualified candidateswill demonstrate superior patient care and possess outstanding communicationskills while adhering to our high standard of excellence.

Duties of a TH Staff Associate/SupervisingCase Manager in the Care Management Department may include the following butare not limited to:


Brief Description ofDuties: 

 

·      Support efficient use of resources.

·      Input into department budget, utilization review, staffing anddepartment systems and process.

·      Provide direction and guidance to staff regarding role andresponsibility.

·      Oversees staff performance including Case Managers and AppealManagers.

·       Input in performance on other disciplines within the department.

·      Support the financial goals and strategic plans of the hospital.

·      Maintains knowledge and competence in Care Management programs,performance improvement activities, staffing analysis, weekend activities anddata collection as directed by the department.

·      Maintain a high level of customer service.

·      All unit coverage as indicated.

·      All other duties as assigned.

 

Qualifications

 

Required Qualifications: NY State RN License and Bachelor Degree.  Five years acute care experience.  Experience in CM or Managed Care. Certified CM. Working knowledge of observation, RAC, Cert,MAC.  Understanding of discharge planningand barriers in the system to discharge. Experience with Unit Rounds and transitional care informationneeded.  Interview skills/high level CSRservice with demonstrated leadership experience.  PRI Certified.  Weekday and weekend flexibility, day andevening rotation as needed.

 

PreferredQualifications:   Certified in Quality/CDI.  Recent tertiary care experience.  Experience in data base systems e.g. excel,word, access and hospital systems. Working knowledge of MCG/Interqual criteria.  Track, trend and analyze reports withdocumentation.  Managed Care or IPROexperience.  Knowledge of regulatoryrequirements i.e. CMS, OBS, Two Midnight Stay, EMTALA, etc. Experience in a Leadership role

This position will remain posteduntil filled or for 90 days max.
