DESCRIPTION

This position will work in UCSD Health's newest program, Population Health Services Organization (PHSO). PHSO supports and work in in collaboration with ambulatory care providers to help with our shared mission of population health for the patients we serve. This NP role will focus on our high risk senior members that are home-bound that would benefit from intermittent home visits in concert with office visits to the patient's PCP to help keep our patient's healthy and safe at home. The Nurse Practitioner (NP) will provide home visits to our home bound rising and high risk Managed Care and managed fee-for-service (MFFS) ACO members. Our goal is to reduce avoidable ED and inpatient hospital stays to minimize the burden this places on our patients and their care givers. This NP role is supported by our team of Nurse Case Managers, Social Workers, Care Navigators and reports to our Chief Medical Officer and work collaboratively with our existing NP in our UCSD at Home program. The geographic area that this position will focus on is greater San Diego, specifically East and South San Diego areas. Travel is required within specific zip codes, flexibility and enjoy working in an innovative patient-centric program is key.

The ideal candidate has previous home visiting experience and/or skilled nursing experience, strong clinical and relational skills, and a passion for visiting seniors in their homes. Our NPs work collaboratively with the patient’s primary care provider supporting our seniors at home with medication management and in home assessments for care planning. Our team consists of RN Case Managers, Social Workers and Care Navigators. The ability to speak Spanish would be an added advantage.

There are two positions available - one focused on home care, one focused on SNF care.

MINIMUM QUALIFICATIONS
  • Must be a graduate from an accredited nurse practitioner program.

  • California Registered Nurse (RN) license, California Nurse Practitioner (NP) license, and Furnishing License Certification.

  • Current NP Specialty Certification from one of the following Nursing Boards: AANP, ANCC, NCC, PNCB, AACN.

  • BLS/CPR certification at time of hire with commitment to get BART within six months of hire date.

  • Minimum of one (1) year of recent working NP experience in a primary care, home care, and/or SNF environment.

  • Clinical advanced practice experience within the last two years gained through school or work experience.

PREFERRED QUALIFICATIONS
  • ACLS and/or ART.

  • Previous experience doing home or SNF visits.

  • Understanding of Medicare and associated guidelines.

  • Bilingual English/Spanish.

  • Well developed clinical competency in general practice setting.

SPECIAL CONDITIONS
  • Must be able to obtain UCSD medical staff privileges.
  • Selected candidate may be hired at the NPIII level pending review and approval by the Advanced Practice Council.
  • Must be able to work various hours and locations based on business needs.
  • Willing to travel and work in a fast paced environment independently.
