Under the direction and supervisionof the licensed Cardiologist, the Nurse Practitioner, in the Lewis Katz Schoolof Medicine’s Department of Medicine, Section of Cardiology’s, Heart Failureand Transplant Program will provide routine and episodic care to complex heartfailure patients in an outpatient environment in collaboration with the HeartFailure Team. 

Responsibilities include: providingrelevant health instruction and counseling, under the supervision of acollaborating physician, to include comprehensive clinical care whichencompasses consultations with longitudinal patient care focused around heartfailure management, pharmacologic intervention and patient education; performingpatient interviews to ascertain medical history and conducting routine physicalexaminations as required; performing assessments and planning to determinetreatment modalities; following-up on referrals to ensure patient compliancewith health care recommendations; working closely with other departments andphysicians to coordinate internal and external ancillary services; assisting inthe development of nursing and nurse practitioner protocols; participating inHeart Failure protocol development and logistics, CardioMEM monitoring, Heartlogic monitoring, out patient diuresis visits at infusion center. In addition,the Nurse Practitioner will lead patient support groups and other heartfailure awareness activities; ordering and interpretingdiagnostic testing and intervening appropriately and completing peer-to-peerworkflow, when necessary. Performs other related duties as assigned.

Required Education and Experience:
Current license to practice as a Nurse Practitioner in the Commonwealth ofPennsylvania

National Certification as aCertified Nurse Practitioner by an accredited certifying body.

Required Skills and Abilities:
*Demonstrated ability to solve complex problems within established policies,and use independent judgment.
*Must maintain continuing education requirements for licensure, and remainup-to-date in area of practice.
*Ability to manage projects.
*Ability to operate all standing office equipment and software including MSOffice.
*Must have strong written and verbal communication skills with the ability tointeract with a diverse population of students, faculty, staff, and patients.

*Must have prescriptive authority.

*Navigation and use of electronicmedical record.

*Knowledge of applicableprofessional principles.

Preferred:
*Experience using EPIC.

*Experience in a Cardiovascular orCritical Care environment in a Nurse Practitioner capacity.

This position will have regularwork hours from Monday through Friday, 8:30 a.m. to 5:00 p.m.
