DESCRIPTIONUnder the direction of the Transplant Social Work Supervisor, candidate is responsible for ongoing pre-transplant psychosocial assessments and post-transplant follow-up of Cardiothoracic transplant patients. Candidate will also provide coverage for the Abdominal Transplant programs as well as living donor/recipient assessments as needed. Candidate will facilitate transplant and caregiver support groups and participate in various transplant committees.
Responsible for providing resources for local housing for out-of-town patients/families. Must Assist in coordination of discharge planning as needed and collaborate with transplant staff and social workers in other transplant programs. Position includes crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. The social worker is responsible for education and advocacy of patients, families and/or significant others. Candidate will participates in related department functions, such as community and program education.

Depending on the experience of the selected candidate, this position will be hired at a Clinical Social Worker II or Clinical Social Worker III level.
MINIMUM QUALIFICATIONSA Master's degree in social work from an accredited school.
At least two years of post MSW experience.
Ability to work collaboratively as a team.
Must have proven independent thinking, decision making, problem-solving and prioritizing skills.
Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients, and external constituents, both verbally and in writing.
Excellent ability to multi-task with demanding schedules.
Experience working as transplant social worker.
Bilingual in English/Spanish.
Licensed Clinical Social Worker (LCSW) issued by the state of California.
Experience working in both inpatient and outpatient hospital setting.
Must be able to work various hours and locations based on business needs.
