DescriptionThe managerof behavioral and social health is a key member of the TRUST Clinic whoprovides a full range of services including counseling, outreach, and casemanagement services to individuals engaging with the TRUST clinic throughTemple University Hospital and in the ambulatory setting. The manager will workdirectly with TRUST’s medical director in overseeing case management servicesfor individuals in the program, seeking partnerships and outreach to addresssocial health needs to facilitate recovery. The manager will work in closeconjunction with the entire team but specifically will oversee Temple’s peerrecovery specialists as they provide individual case management and carelinkages for patients in the inpatient and ambulatory settings.The manageris responsible for: Participating in screening/intake for all newpatients to the TRUST clinic with particular attention to social determinantsof health and trauma history, providing education, social services, and casemanagement services to program patients, assisting patients in applying foridentified social needs (ie housing, education, food, transportation), developinglinkages and new programs to facilitate social services needs for programpatients (ie vocational training and skills development), overseeing anddeveloping peer recovery specialist program in inpatient and ambulatory settingincluding recovery meetings and individual case management by peer recoveryspecialists, maintaining accurate patient records, providing patients and/orfamily members with information about addiction issues and about services andprograms, making appropriate referrals when necessary, completing other relatedduties as assigned RequiredEducation & Experience: BA/BS Degreein social work, psychology, or service-related field, or experience equivalency,and at least 4 years of directly related experience. Knowledge of harmreduction philosophy, patient-centered care and motivational interviewingtechniques, and trauma-informed care. An equivalent combination of educationand experience may be considered.
