Duke Connected Care, a community-based, physician-led network, includes a group of doctors, hospitals and other healthcare providers who work together to deliver high-quality care to Medicare Fee-for-Service patients in Durham and itssurrounding areas.

This position will support expansion of and ongoing enhancements to Duke Health’s population health management platform, which aims to improve health outcomes, lower unnecessary healthcare costs, enhance patient satisfaction. Individuals in this position will provide administrative direction in the design, development, implementation, monitoring, and continuous improvement of population health management programs to ensure PHMO fulfills its program objectives as well as regulatory and contractual requirements.

Program Development, Implementation and Integration -

Play key role in ensuring new programs/initiatives and program enhancements are effectively vetted, planned, designed, and implementedCoordinate and participate in meetings with PHMO leadership and other population health champions to determine program objectives and metrics; identify internal and external resources available to meet those objectivesEngage and collaborate with and across PHMO teams (e.g., Provider and Patient Engagement, Care Management Quality, Data and Analytics) to leverage internal resources and synchronize effortsCoordinate program requirements, develop and manage project plans, assemble and coordinate cross-functional project teams to execute plansCoordinate vetting process for non-contractual program proposals originating both internally (PHMO) and externally (Duke/CIN), including interviewing party initiating proposal, completing intake form (SBAR), and presenting summary to PHMO Clinical Programs Oversight CommitteeIdentify and help resolve barriers to progressPrepare and document program workflows, protocols, procedures, and operating policies and contribute to staff training to ensure successful transition from program development to implementationEstablish and maintain lines of communication within project teams and with stakeholders and championsCoordinate the compilation of and prepare operational reports and analyses setting forth progress, adverse trends and appropriate recommendations or conclusions.


Annual Quality Abstraction –

Oversee annual MSSP Quality reporting processDevelop expertise on all measure specifications and keep up to date on CMS quality information and resourcesDevelop staffing plan in conjunction with other PHMO teamsDevelop and manage project plan, working closely with Medical Director Team, Lead Pharmacist, Data Team and Patient Services Teams to ensure reporting is delivered on time and with the highest degree of accuracy and integrityWork with Duke Compliance Office to coordinate internal auditIdentify opportunities for year-over-year process improvement with input for all participantsCommunicate incremental progress to leadership and develop/present high-level status reports for leadership and governance groups


Quality Performance –

Contribute to quality performance improvement strategic planning based on knowledge of current performance, strengths and opportunities within portfolioCoordinate full lifecycle of subset of annual QI projects, employing program development/implementation responsibilities outlined belowTrack incremental quality performance using internal reporting methods and data from payersCommunicate progress toward quality targets to leadership and develop/present high-level status reports for leadership and governance groupsIdentify threats to high performance and suggest mitigation strategies


Payer Engagement –

Perform account management for a defined portfolio of payersDevelop expertise on payer contracts and productsServe as primary point of contact for assigned payers; play proactive role in bidirectional communication and data exchange; manage payer expectations regarding delivery of servicesWork closely with Data Team and Gap Closure Team to ensure timely progress toward program goals; monitor performance against contracts and develop plans for corrective action as neededContribute to comprehensive payer engagement strategy, including developing and disseminating best practices


Minimum Qualifications

Education

Work requires communications, analytical and organizational skills generally acquired through completion of a bachelor's degree program.

Experience

Work requires the ability to plan and administer programs and direct program activities within a specific functional area, generally acquired through four years of related experience. OR ANY OTHER EQUIVALENT COMBINATION OF RELEVANT EDUCATION AND/OR EXPERIENCE

Additional Competitive Qualifications:

  • Master’s degree preferred
  • Advanced project management and organization skills
  • Project Management and/or Business Analyst certification a plus
  • Strong communication skills (written, verbal, listening)
  • Ability and desire to work in a collaborative environment
  • Analytical mindset
  • Advanced skills in Excel and PowerPoint
  • Proficiency in Visio
  • Proven ability to handle multiple projects toward effective solutions and according to budget and timelines

    Duke University was created in 1924 by James Buchanan Duke as a memorial to his father, Washington Duke. The Dukes, a Durham family that built a worldwide financial empire in the manufacture of tobacco products and developed electricity production in the Carolinas, long had been interested in Trinity College. Trinity traced its roots to 1838 in nearby Randolph County when local Methodist and Quaker communities opened Union Institute. The school, then named Trinity College, moved to Durham in 1892, where Benjamin Newton Duke served as a primary benefactor and link with the Duke family until his death in 1929. In December 1924, the provisions of indenture by Benjamin’s brother, James B.
