The Social Worker, MSW for the Case Management Department under the direction of the Director of Case Management. The Social Worker assesses, plans, and evaluates the needs of patients for discharge planning. The Social Worker will develop discharge plans according to the needs of the patients including but not limited to: determining the eligibility for social programs and funding sources, assessing whether patients qualify for community assistance, and creating referrals for post hospitalization needs. The Social Worker shall assist the patient/family in developing a care plan as it relates to his/her medical problems. Discharge planning is coordinated with physician, nursing, case management, social worker and any other medical discipline who has an ongoing medical relationship with the patient. They shall also assist with transfers to other acute care facilities, short term and long term placements, as well as assist with appropriate home health referrals. Any combination of relevant education, work experience, and / or certifications will be considered
- MSW degree and BLS is required.
- One (1) year in hospital case management discharge planning or related medical field with post facility care coordination of services.
- A good working knowledge of patient discharge needs community resources and placement process. In-depth knowledge of community resources and referral requirements.
- The ability to adapt and facilitate change.
- Computer knowledgeable to facilitate timely education and retention on computer usage for documentation, referrals and purposes of e-mail use.
