DESCRIPTION

Under general supervision, the Care Manager is responsible for facilitating the patient's hospitalization from pre-admission through discharge. The Care Manager coordinates with physicians, nurses, social workers and other healthcare providers to expedite medically appropriate cost effective care. The Care Manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. The Care Manager will communicate with administration and the healthcare team as needed to facilitate patient care.

The ED Care Manager works with the ED physician to ensure patients are in the correct status upon admission. The ED Care Manager focuses on the alternative discharge strategies for those patients not meeting acute level of care needs, and works with Social Workers to provide appropriate community resources and follow up care to prevent unnecessary readmissions to the hospital and inappropriate utilization of the ED.

MINIMUM QUALIFICATIONS
  • Must be a graduate from an accredited school of Nursing.

  • Current Registered Nurse (RN) license issued by the State of California.

  • BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.

  • Minimum three years of acute care RN experience.

  • Minimum one year of acute case management experience. Will consider a minimum of three years of recent ED RN experience in lieu of acute case management experience.

  • Proficiency in Information Systems, databases, and computer programs including MS Office: Word, Excel, Outlook, PowerPoint, etc.

  • Demonstrated knowledge of payer industry, resource management, reimbursement, and evidence-based clinical practice.

  • Proven ability to construct grammatically correct correspondence and reports using standard medical terminology.

  • Excellent relationship and management skills, including a high degree of psychological sophistication and non-aggressive assertiveness.

  • Demonstrated ability to problem solve, engage in abstract thought, and successfully manage conflicts.

  • Strong negotiation, organizational, delegation and task prioritization skills.

PREFERRED QUALIFICATIONS
  • BSN - MSN.

  • ACM and/or CCM certificate.

  • Previous utilization management or revenue cycle experience.

  • Experience working collaboratively with a diverse team consisting of physicians, nurses, finance, billing, etc.

  • Previous Supervisory/Management experience.

  • Experience with Epic, Midas, ECIN, Interqual.
