Position Summary

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings.  Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with:  mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement.

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Duties of a Social Worker II may include the following but are not limited to:

  • Charting responsibilities
  • Team discussion/rounds
  • Discharge planning
  • Patient/Family Assessments
  • Outside agency contacts
  • Insurance and hospital contacts regarding admissions\\
  • Float to all behavioral health araes including Mobile Outreach Team
  • Qualifications:

     

    Required Qualifications MSW from an accredited Social Work Education School. NYS LMSW or LCSW.  Experience in Psychiatric Health Care. Demonstrates ability to be trained in child psychiatry for purposes of coverage as needed. Demonstrates excellent written and oral communication skills. Ability to work and coordinate with established Psychiatric Medical Treatment Team. Demonstrates understanding of discharge planning and Long Length of stay barriers and systems. Demonstrates understanding of Psychiatric specific community resources. Demonstrates good assessment and psycho-social skills. Working knowledge of Long Island and Metropolitan NY community resources. Demonstrates patient first, patient advocacy and service excellence.  Computer literate.  Demonstrates understanding of community systems.  Demonstrates ability for project management and action planning.  Demonstrates team work and team leader focus.  Demonstrates understanding of data collection and statistics.

    • Preferred Qualifications:  
    • Experience in inpatient Psychiatric care.  Bi-lingual.
    • 3 yrs experience in a hospital setting
    • Emergency Department experience
    • Experience in substance abuse/counseling intervention
    • Experience in medicine SW with Psychiatry collaboration
    • Psych/Medical Integration
    • Experience in behavioral health
