Position Description

The person in this position provides comprehensive substance use treatment to adults living with HIV infection receiving medical services in the UNC Infections Diseases Clinic. Additionally, provision of general social work, medical case management, mental health, and referral services will be expected. A much smaller percentage of time will involve provision of services to patients with other infectious diseases in collaboration with members of the medical and social work teams. The person in this position is responsible for coordination, planning, and implementation of the substance use treatment plan including a wide range of patient centered services that facilitate linkage to and retention in HIV and associated medical care. Services may be delivered in individual and/or group settings. Services may include but are not be limited to comprehensive psychosocial and substance use assessment, medical case management, mental health and counseling services, assistance with transportation, housing, nutrition, financial or other needs. Person in this position will work closely with medical providers, and other interdisciplinary team members including physicians, advance practice providers, nurses, social workers, pharmacists, financial counselors and others. Employee must be able to collaborate with other community and social service agencies to facilitate services for patients. The person will play an active role in program development and clinical quality improvement initiatives in the clinic. Employee must have ability to effectively communicate with patients, their families and support system to promote compliance with treatment plans directed. The person in this position will be utilizing all types of communication, such as a variety of platforms (EMR communication, texting, email, phone calls, etc.) in communicating with faculty, staff, patients, their support system, and their families to promote achievement of treatment plan goals.

Role requires timely charting, care planning, data entry and data management in patient’s electronic medical record (EPIC) and other electronic databases. A strong ability to multi-task in a very fast-paced environment is essential. Person in this position is required to attend monthly staff and quarterly provider meetings and other local meetings as needed or directed by supervisors. Must maintain required certifications and update continuing education.

Minimum Education and Experience Requirements

Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.

Essential Skills, Knowledge and Abilities

Must possess excellent interpersonal and communication skills, both oral and written, which are required for interacting with a large interdisciplinary team. Must act within a clinical social work team environment, providing back-up for colleagues when needed. Candidates must have the ability to analyze and interpret information needed to plan appropriate interventions. Essential that applicants have sensitivity to and are respectful of people living with HIV/AIDS, as well as racial and gender/sexual minorities. Must possess knowledge of effective social work principals and practices, policies and resources and have the ability to navigate patients and families through institutional systems (department of social services, health departments, medical home, etc.). Applicants should have the knowledge or experience in counseling patients in medical settings. Ability to communicate effectively in written and verbal formats. Must maintain a valid and appropriate driver’s license and safe driving record.

Preferred Qualifications

Preference will be given to those who have worked in the HIV field. Spanish speaking bilingual candidates encouraged to apply. Prefer candidates with a Master of Social Work (MSW) degree. Experience with electronic medical records and data management preferred.

Required Licenses/Certifications

Certified as a licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board. Certification as a Licensed Clinical Addiction Specialist.

Special Physical and Mental Requirements

Must be comfortable working in a fast paced clinic environment.
