DESCRIPTION

At the direction of the department manager, the Care Management Supervisor shares responsibility for day to day operation of the care management department including direct supervision of the staff. Facilitates patient centered care, interactive management and collaboration within the care management department, interprofessional team and the community that serves our patient population.

The Supervisor has primary responsibility facilitating patient flow. They demonstrate expertise and the ability to implement timely transitions of care with plans that are safe, effective and focused on preventing unplanned returns to the ED and or hospital. This role models the mission, vision, and core values of the organization; supports evidence based practice to insure optimal outcomes.

MINIMUM QUALIFICATIONS
  • A Bachelor's of Science Degree in Nursing or Master of Social Work.

  • Registered Nurse in the State of California OR License Clinical Social Worker (LCSW).

  • A minimum of five or more years of relevant experience.

  • Experience and proven success in ability to effectively supervise a team and to manage the complex workflow and multiple priorities involved with care coordination and case management.

PREFERRED QUALIFICATIONS
  • MSN

  • Accredited Case Manager or Certified Case Manager (ACM or CCM).

  • Clinical Nurse Specialist (CNS).

  • Certified Professional in Healthcare Management (CPHM).

  • Experience in an acute care hospital setting.
