Position Summary


At Stony Brook Medicine, The Quality Management/SB Safe Practitioner will function as a member of Stony Brook  University Hospital’s Quality Management Program.  The Program has as its goal the promotion of positive patient outcomes through integration of performance improvement activities and measurement tools.


Qualified candidates should demonstrate excellent intrapersonal skills and time management skills.  They should have attention to detail and be able to work with all levels of the organization and they must be able to keep Quality Management data confidential.


Duties of a Quality Management Practitioner may include the following but are not limited to:


  • Monitors the reporting of patient events on assigned services and works collaboratively to ensure appropriate follow-up.
  • Identifies, investigates and interprets patient event data trends for the purpose of improving quality and safety in patient care.
  • Identifies case review triggers, practice issues and trends and collaborates with Nurse Managers, Clinical Nurse Specialists, Unit educators and nursing staff to initiate action plans for process improvement.
  • Identifies and navigates the dissemination of physician case reviews within the patient safety reporting system to ensure adequate and timely case evaluations.
  • Acts a facilitator for performance improvement teams, including coordinating efforts with key stakeholders such as physicians, nurses, ancillary staff and Information Technology.
  • Act as a quality management liaison to High Reliability Unit Teams, Program of Distinction (POD) Committees and Performance Improvement Committees as needed, providing patient event reporting data to hospital wide, service and unit based quality venues.
  • Train staff for facilitator role and provide education to staff and performance improvement teams on the principles and tools employed in performance improvement activities including Lean and Six Sigma.
  • Assist in the coordination of Joint Commission and Department of Health activities related to the performance improvement standards.
  • Assist in the development of POD/service line report cards and physician profiling, analyze and aggregate data, and facilitate improvement initiatives.
  • Provide expertise for clinical performance improvement projects related to patient safety, patient outcomes and disease management.
  • Facilitate root cause analyses and failure mode and effect analyses.
  • Involvement with clinically-driven quality improvement initiatives relating to clinical service/other quality-related group activities.
  • Assist in achieving goals related to departmental performance standards.
  • Implement institution-wide initiatives in order to conduct hospital business.

Qualifications


Required Qualifications:  Baccalaureate in Nursing.  3 years relevant experience using CQI principles including Lean or Six Sigma to improve performance. Experience in data collection tool development and statistical analysis is required. Knowledge and application of performance improvement methodologies is crucial. Must be able to effectively interact and communicate with department heads/supervisors on a house-wide basis. Proficient in MS Office, including MS Word, Excel, Outlook, Visio.


Preferred Qualifications:  Master’s degree in related field. Training experience and demonstration of good presentation skills are preferred. Certification in QI and/or Lean and/or Six Sigma.
