About the UnitThe Extension for Community Healthcare Outcomes Chicago (ECHO-Chicago) is an innovative model that uses advanced communications technology to bring together academic medical center expertise with local primary care providers to expand access to specialized care in Chicago's underserved communities. UC specialty care providers host a series of 10-15 sessions, delivered via high-quality videoconferencing technology, designed to train primary care providers to manage complex chronic diseases and conditions such as hypertension, ADHD, hepatitis C, adolescent health, childhood obesity, childhood adversity & trauma, pediatric asthma, pediatric obesity, geriatrics, geriatrics for skilled nursing facilities, behavioral health, depression and anxiety, opioid use disorder, serious mental illness, managing patients at high risk for hospitalization and women's health. ECHO-Chicago is based in the Department of Pediatrics and works to address issues of healthcare access in adults and children living in underserved urban communities.

Role ImpactIndividual Contributor

ResponsibilitiesThe job manages academic, research, clinical or administrative programs of the University.

1) Uses moderate/solid understanding and experience to administer the delivery of services to program participants and/or beneficiaries., 2) Sets and communicates program priorities and performance standards and assesses operations using these criteria. Plans and conducts quality assurance reviews and recommends changes as appropriate., 3) Analyzes program budgets and recommends or makes budgetary recommendations., 4) Has moderate/high levels of interactions with faculty, researchers and staff for committee work or information., 5) Performs other related work as needed.

Unit-specific Responsibilities

1) Responsible for task leadership and sub-task management for telehealth sessions, collaborating with facilitator to schedule sessions, and working with the team to finalize evaluation tools.

2) Coordinate telehealth sessions and provide pre- and post-session administrative support for program facilitators and community providers related to the telehealth sessions.

3) Provide technical assistance to community providers for participation in telehealth sessions. Manage communication with community partners and providers. Facilitate the collection and management of project related data from participating community partner sites.

4) Provide administrative support including documentation of session attendance and activities, sending meeting reminders and session’s summaries, maintaining calendars, and providing other logistical support to the ECHO-Chicago project.

5) Provide ongoing support for new cohorts participating in series, including working with facilitator to schedule series, revising curricula, case templates, and data collection instruments.

6) Provide on-site and/or telephone support (e.g., inquiries, requests for assistance with case presentations, technology equipment, etc.) to community partners.

7) Monitor data flow into respective databases and ensure that collected data are complete, current and valid.

8) Maintain project timelines to ensure projects are completed on time and in alignment with program mission and goals.

9) Interact with internal, community, and strategic partners in a professional manner and respond quickly to inquiries and requests. Help foster teamwork to complete designated tasks.

10) Work with Recruitment Lead to develop marketing materials like monthly electronic newsletter, series brochure, annual report and flyers.

11) Explore potential of new partnerships with community health centers to enhance participation in ECHO-Chicago series.

12) Travel to the community sites, as needed, to develop and foster partnerships with community health clinics and strategic partners.

Education, Experience, and CertificationsMinimum requirements include a college or university degree in related field.

Minimum requirements include knowledge and skills developed through 2-5 years of work experience in a related job discipline.

1) At least 2 years of experience in project coordination, planning, and/or management

2) Some experience in the health or human service field

3) Experience working with a multi-disciplinary team

4) Some experience working with and developing relationships with external partners

5) Demonstrated experience in developing marketing materials, preferably using Microsoft Publisher or Adobe Illustrator

6) Experience working with a web-based data management tool (e.g. REDCap, Survey Monkey, Google Forms, etc.)

7) Valid driver’s license required. Access to an insured vehicle
