Primary Purpose of the Organizational Unit

The Department of Social Work’s primary purpose is to educate students and the signature pedagogy is experiential learning through field instruction. In order to facilitate field locations, the Department of Social Work develops multiple community partners. Services are provided as students learn about social work.

Position Summary

This position will be based in the Guilford County Department of Public Health and will focus on assessment, diagnosis, and intervention along with linkage and coordination of services for people who have or are at risk of having hepatitis C. The Bridge Counselor will work with multiple systems of care to ensure that people are screened and assessed both for Hep C and other co-morbid concerns that may include mental disorders and/or substance use disorders. Additionally, the Bridge Counselor will link and coordinate services for people, particularly those who are positive for Hep C.

Minimum Qualifications

Master’s degree in social work from an accredited program and the Licensed Clinical Social Work Associate credential. Some experience in working with people who have substance use disorders and other mental disorders.

Preferred Qualifications

Prefer lived experience with substance use and in-depth familiarity with recovery concepts.

Conduct screening and assessment relative to health and behavioral health

Essential Tasks

Bridge Counselor will engage potential clients in a screening and assessment process that includes current physical and behavioral health concerns as well as substance use concerns. Appropriate screening and assessment tools will be used to assist in this process.

Facilitate testing for Hep C

Essential Tasks

Bridge Counselor will work with an interdisciplinary team to facilitate screening and testing for hepatitis C and other communicable diseases. The Bridge Counselor will be the key person in this coordination effort.

Provide diagnosis and brief intervention if applicable

Essential Tasks

The Bridge Counselor will use data from screening and assessment to assign appropriate diagnoses based on the current version of the DSM. Additionally, the Bridge Counselor will formulate brief intervention plans based on the diagnoses, needs, and strengths of the person served.

Provide linkage and coordination of appropriate services

Essential Tasks

The Bridge Counselor will provide linkage and coordination of needed services in order to facilitate appropriate care for the people served by this project. Services include phone calls, meetings, and other electronic information sharing, with appropriate permission from the person served.
