Nurse Navigator, Care Connect Program, University Advancement

Reporting to the Senior Nurse Navigator, the Incumbent is responsible for the coordination and follow-up on implementation of a care plan for an assigned group of patients to ensure seamless care throughout the hospital, ambulatory practices, and varied and complex healthcare disciplines. Incumbent serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care, and coordinates care with the physicians and members of the healthcare team. This position may require off-site visits, such as to a patients home, and working during nights and weekends to assist patients. The Nurse Navigator will provide access and coordination for a variety of patient groups.

Salary: Commensurate with experience
Total Hours: 8-5, M-F
