The responsibilities include, but are not limited to: Administrative

• Coordinate regular meetings including multidisciplinary fetal meetings with patients and providers, maternal fetal medicine monthly meetings, and meetings to establish new clinical practice guidelines/protocols

Direct/Indirect patient care

• Serve as coordinator for the single ventricle home monitoring program, including providing educational materials, pursuing new technology such as electronic recording of data, coordinating discharge for each patient (home health, oximeter, home oxygen, CPR training, medication list, follow-up appointments, etc.); develop single ventricle program and expand into technology-based program
• Follow patients weekly/bi-monthly with primary cardiologist, communicating with primary care physician and other disciplines the specialty care of patient, calling home health agencies and communicating their monitoring responsibilities, and serve as contact for the needs of the families
• Coordinate surgical process for each pediatric surgery patient including consult, pre-op visits (labs, chest x-rays, and blood products), inpatient stay, discharge and post-op follow-up; round daily with cardiology and cardiothoracic surgery teams; complete all discharge education for post-operative patients and order discharge medications for patients; ensure all aspects of inpatient stay (operative notes, discharge summaries, medication list) is delivered to referring agencies and primary care providers
• Coordinate care with other specialties to ensure limited travel for patient care/office visits; travel to outside offices to bring awareness of the Pediatric Cardiothoracic Surgery Program at the Children's Hospital of Georgia; serve as urgent referral contact for providers; advocate for patients for funding, education materials, and further research
• Attend (when available), morning/afternoon pediatric heart team rounds; attend weekly multidisciplinary Cath conferences
• Perform all other related patient care duties as assigned
