About the Unit
The Chicago Center of HIV Elimination, embedded within the University of Chicago's main campus in Hyde Park, is located in Chicago's HIV epicenter. Of the 20 neighborhoods in the city with high annual HIV diagnosis rates (greater than 40 per 100,000), 13 of them are no more than eight miles from the University of Chicago. This area includes 4 of the 6 communities with the highest annual diagnosis rates in the city (Washington Park, West Englewood, Greater Grand Crossing and South Shore). Our position within the hardest hit neighborhoods in Chicago provides unique opportunities to advance HIV testing and prevention interventions locally, providing tangible results to those most affected and to improve the lives of those living with and without HIV infection. CCHE seeks to eliminate new HIV transmission events over the next 30 years (from 2011 to 2041) by using network science to target and integrate prevention as well as create structural and community-specific interventions.

Job Information

Job Summary:

The candidate will provide HIV testing, result notification, and linkage to care for individuals seeking HIV testing at CCHE or the associated community space. This worker will implement HIV testing, linkage, and retention programs for the Chicago Center for HIV Elimination. They will engage individuals at increased vulnerability in HIV/STI testing, HIV and PrEP care, resource counseling and other engagement activities at the community space. They will work with the CCHE research team, Manager of Network Services, Director of Prevention Services, supportive services team and Principle Investigator. They will be expected to establish connections with members of high-risk networks and attempt to test, link, and engage these members in care.

Responsibilities:

• Pre-test and post-test counseling.
• Elicitation of sexual and social networks; provision of results; and linkage to care.
• For linkage to care the candidate will be responsible for facilitating the scheduling of appointments for participants; attending medical care appointments with patients; tracking and reporting patient outcomes and visit attendance; coordination with CCHE research and service programs; and providing brief client-centered case management.
• Provide partner notification to those individuals who are identified through the network elicitation process.
• Provision of services may also include: linkage to HIV and PrEP care; referrals to other community based organizations (CBOs) and social service agencies (e.g. housing, food, and primary care organizations); follow up with participants of the program; monitoring and evaluation of services; development of community outreach programs and procedures; documentation of client encounters and services.
• Assist in the development of protocols and procedures for the network based HIV/STI testing program and linkage to care; and actively pursue required knowledge and skills for professional development.

• Ability to travel to various community venues across the metropolitan area required.
• Ability to perform phlebotomy or a willingness to be trained in phlebotomy required.
• Ability to work on multiple projects required.
• Ability to work non-traditional hours required.

Additional Requirements

Education and Experience:

• High school diploma or equivalent required.
• One year of work experience providing testing and result notification is required.
• CDPH/IDPH HIV test counseling certification preferred.
