Advertised Job Summary: The purpose of this position is to provide and monitor a clinical case management system that provides continuity of care and follow up for clients on an urban university campus of approximately 35,000 students. This person will coordinate and support the needs of clients through an interdisciplinary team of counselors, psychologists, physicians, nurses, and other providers by assisting with follow up care. Duties will include, but are not limited to clinical reporting; tracking no-shows/cancellations and high-risk clients; scheduling referrals; obtaining authorization to release and obtain client information and/or medical/psychotherapy records; assisting and tracking caseloads; coordinating hospitalizations and/or post-hospitalization care; providing education, emotional support, preventative care, and health promotion; tracking interventions and outcomes; providing triage, assessment and crisis intervention as needed. This person will function as the liaison between client referrals, providers, and external partners by providing follow up information; arranging appointments; reviewing alternate resources and facilitating connections to care; establishing care plans, education and discharge disposition. This person will initiate and develop ongoing liaison relationship with community providers, services, hospitals, and other resources and maintain and update a database of community providers including regional and national resources for specialized care. They are responsible for assisting with mental health initiatives and community based services, as well as training to staff on case management systems. They will practice in accordance with ethics of their applicable professional organization such as the National Association of Social Workers, American Counseling Association, American Psychological Association, as well as state and federal law related to medical/therapeutic service provision, and departmental guidelines and policies. Credentialing, privileging and peer review are required for this position. Position is considered mission critical and must report during campus emergencies.
Minimum Qualifications (Classified and Unclassified positions): Master's degree in a related field and 3 years related experience, or bachelor's degree and 5 years related experience. Professional License (social work, counseling etc.), or eligible within 6 months of employment.
