Job description



The Patient Navigator is a grant-funded, one year position. This person will work with research participants and caregivers to facilitate communication between physicians, clinic staff and all other care providers in the continuum of care in order to identify strategies and resolve barriers to Hepatitis C.

Shift

Primarily Monday-Friday 8AM-4:30PM.



Skills/Knowledge/Abilities

Use of electronic medical record, good problem solving skills. Excellent communication and customer service skills. Knowledge of and experience working with community based resources providing services to the identified patient population.

Previous experience in education and health care preferred.
