Position Summary:

Incumbent provides patient care to an assigned group of patients in the Senior Center. The nurse incorporates the psycho-motor and age appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation. Assists with gathering, entering, and evaluating data related to departmental DSRIP projects. Acts as a role model to nursing students and other staff in the unit. Utilizes a team based approach to delivering patient care services.

Salary Range:

Rate Minimum $55.92 Midpoint $55.92 Maximum $55.92

Required Qualifications:

Ability to establish and maintain effective working relationships across the HealthSystem

Ability to maintain a work pace appropriate to the workload

Ability to manage and prioritize multiple requests and priorities

BLS certification upon hire

Critical thinking skills to problem solve and communicate with physicians on clinical issues

Current California RN license

Demonstrated experience using a computer, including data entry and retrieval

Demonstrated experience working in a geriatric acute care or clinical setting

Excellent written and verbal communication skills in English

Minimum of six months demonstrated nursing experience

Must demonstrate customer service skills appropriate to the job

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties

Preferred Qualifications:

Bilingual in English and Spanish

Knowledge of University and medical center organizations, policies, procedures and forms

Prior experience coordinating patient care utilizing tele-monitoring equipment
