Nurse Clinical Manager Sr/UKHC

Job description

This person will work with Family and Community Medicine-Turfland leadership to ensure healthcare transformation goals are met for chronic care management (CCM) using PCMH principles, valued based care and the DFCM strategic plan.

The RN Care Manager Supervisor will work with Family and Community Medicine-Turfland leadership in developing and implementing plans for process improvement, practice innovation and/or quality improvement in all areas related to chronic care management.

This employee must demonstrate knowledge of complex concepts relating to transformation and innovation and will use their expertise in value-based care, alternative payment models, quality improvement, health IT and change management to assist with improving quality and value performance across a variety of public and private payer programs such as patient centered medical home (PCMH), pay-for-performance, MACRA, MIPS, and Alternative Payment models.

Physical Requirements

The ability to lift, carry, push, and pull 50-100 pounds on a daily basis; as well as stand, walk and sit for long periods of time. The conditions of work may involve exposure to communicable diseases and injury from intractable patients or sharp instruments.


Monday-Friday 8:00am-5:00pm, may require some holidays


Nursing, management, EMR.
