Additional Information Purpose To provide professional social work functions and serve as an integral member of a broader team involving physicians, nurses, medical assistants, and investigators in a interdisciplinary clinical Center. Essential Functions Assist patients with applications and documentation for Social Security disability claims, Medicare, protected work time, assistive devices, referrals, and other related areas. Help facilitate timely communication and effective coordination regarding the interdisciplinary plan of care, patient and family education and payer relations. Organize caseload so that necessary case records and documents are processed and updated within specific time limits established by regulation and state, federal, and local policy. Assist the patient and family with identifying and accessing community resources. Assess and identify the patient's psychosocial and support systems issues and suggest updates to plan of care as appropriate. Consult with collaborating staff on patient management issues. Evaluate patient outcomes and adherence to care plans. Provide patient and family education tailored to the specific needs of each patient. Marginal/Incidental functions Other related functions as assigned. Required qualifications Bachelor's of Social Work degree and a minimum of 5 years as a professional clinical social worker in an outpatient clinical practice. Ability to work efficiently and flexibly in a fast-paced environment with minimal guidance. Excellent written and verbal communication skills. Outstanding organizational and time-management skills. Ability to address individuals and information with a high degree of sensitivity and confidentiality in various delicate, formal, or urgent situations. Ability to develop and maintain effective working relationships with physicians, staff, and patients. Must be comfortable and flexible with changing priorities and self-directed problem solving. The retirement plan for this position is Teacher Retirement System of Texas (TRS), subject to the position being at least 20 hours per week and at least 135 days in length.
