Case Management Coordinator
Northern Arizona University (NAU) has an opening for a full time, 12-month Case Management Coordinator (CMC), a member of a vibrant team of interdisciplinary professionals within the integrated care setting of Campus Health Services (CHS). This position will be responsible for providing ethical, confidential, and culturally informed services in support of NAU students while performing a variety of duties in supporting Campus Health Services. This position reports directly to the Director of Counseling Services.
The successful candidate must possess a doctorate or masters in social work, psychology, or counseling and be committed to social justice values. The CMC supports all employees of CHS by identifying, reporting, and managing circumstances regarding high-risk mental health concerns involving NAU students for campus partners. Additionally, this specialized position functions as the primary contact for campus departments and community mental health agencies in coordinating care. The CMC provides advocacy for students in need through fostering collaborative relationships with key campus constituents and engaging in regular consultation with off-campus providers, including Flagstaff Medical Center’s Behavioral Health Unit, The Guidance Center, etc.
- Build and maintain collaborative relationships with various University departments and community agencies (i.e., Office of Student Life, Housing and Residence Life, NAU Police Department, Campus Health Services, etc.).
- Implement, manage, and maintain a case management system to coordinate, facilitate, and monitor programs, services, resources, and referrals available to NAU students.
- Participate in weekly NAU Student Behavioral Intervention Team (SBIT, meeting with key campus constituents. Maintains notes and documentation from meeting in Electronic Health Record).
- Monitor at-risk student compliance with treatment plans and/or University behavioral expectations.
- Review daily police blotter, Electronic Health Record (EHR), and applicable Incident Reports daily to scan for potential mental health related issues and documentation.
- Assist in the identification and management of culturally appropriate and inclusive on-campus and off-campus resources necessary to intervene with students of concern, with goal of reducing the risk or threat presented.
- Present at-risk cases to CHS’s Psychiatric Case Consultation team every other week.
- Track hospitalizations of students for mental health reasons and connect with case managers at local in-patient units in order to facilitate communication regarding intake, discharge planning, and crisis intervention.
- Routine evaluation of the case management system to ensure quality of care and participation in quality improvement efforts designed to optimize the efficacy of services provided as requested.
- Maintain updated directory of local community mental health providers, organizations, and resources, and assist mental health and medical providers, university departments, and NAU administration with referrals for students when beneficial.
- Provide outreach for Counseling Services to maintain and enhance student/faculty/staff connection to mental health supports on campus.
- Represent Counseling Services on committees and at various programs, meetings, or activities as needed.
- Direct Supervision of Case Management Graduate Assistant.
- Provision of direct service to NAU students in the form of supportive contact, individual therapy, group therapy facilitation, and/or outreach.
- Other duties as assigned
- Doctorate or Master’s degree in a mental health field (social work, counseling, psychology) from an accredited institution.
- License or license-eligible as a social worker, psychologist, counselor in the state of Arizona at the time of hire, or within 12 months of start date.
- Demonstrated competence in culturally informed practice.
- Excellent written, verbal, and time management skills.
- Demonstrated skill at developing and fostering strong professional and supportive relationships with both on-campus and off-campus resources.
- Demonstrated multicultral competency
- Experience with triage/crisis intervention
- Coordination of care to college aged population
- Three years post-degree experience in case management
- Experience with Electronic Health Records and databases
- Experience providing supervision
Knowledge Skills and Abilities
- Demonstrated ability to work collaboratively with multidisciplinary colleagues, internal and external agencies, individual clients (in case management), and families.
- Knowledge of case management theory and practice in an integrated health care setting.
- Familiarity with psychiatric diagnoses and medications.
- Ability to manage a full caseload.
- Ability to work with people from a variety of culturally diverse backgrounds.

This position has been identified as a safety/security sensitive position. Therefore, per AZ Revised Statute, Northern Arizona University requires satisfactory results for the following: a criminal background investigation, employment history investigation, degree verification (in some cases) and fingerprinting.

Finally, each year the Northern Arizona University Police Department releases an annual security report. The report is a result of the federal law known as the Clery Act and contains policy statements that address the school’s policies, procedures and programs concerning safety and security including policies for responding to emergency situations and sexual offenses. The report contains three years of data for Clery reportable crime statistics for the campus along with the most current year’s Fire Safety Report, which includes policy statements and fire statistics for Flagstaff on-campus student housing.

**The starting salary will be determined by the qualifications of the selected applicant balanced with departmental budget availability, internal salary equity considerations, and available market information.
This position is exempt from the overtime provisions of the Fair Labor Standards Act (FLSA) and therefore will not earn overtime or compensatory time for additional time worked.
