Hematologic Malignancies & Cell Therapy

POSITION:
Advanced Practice Provider - Acute Care for patient with Hematologic malignancies and those going through stem cell transplant.

QUALIFICATIONS:
• Nurse Practitioner Licensure and Certification
• Licensed in the State of North Carolina
• Hematological Malignancy Experience Preferred

POSITION SUMMARY:
Provide direct patient care in the inpatient/day hospital setting to patients during the pre-, peri- and post-chemotherapy/stem cell transplant periods in collaboration with Hematology/ABMT attending physicians and nursing staff. Ensure continuity of patient care. The age-specific scope of practice is between 16 years and older.

RESPONSIBILITIES:
• Perform necessary diagnostic and therapeutic procedures (within the scope of practice of the supervising physician). These may include, but are not limited to the following:
- bone marrow biopsy/aspirate
- bone marrow harvest
- lumbar puncture diagnostic and therapeutic
- hickman catheter removal
- skin biopsy
• Conduct physical exams, obtain patient histories, monitor patient progress
• Order appropriate medications, laboratory studies and diagnostic procedures. Prescriptive practice will exclude chemotherapy.
• Document the patient's course of treatment at the direction of the supervising physician:
- History and Physical
- Problem List and Plan of Care
- Daily Progress Note
- Procedure Note
- Interval Notes
- Discharge Summary
- Discharge Planning
• Accept scheduled and unscheduled inpatients
• Facilitate daily rounds in both the inpatient and outpatient setting
• Collaborate with other program and non-program services, physicians and nursing staff in order to provide a longitudinal continuity of care between the inpatient and outpatient setting, and through follow-up
• Participate in meeting the educational requirements of students, medical residents, fellows and other members of the health care team
• Collaborate with members of the health care team in helping to assure compliance with research related protocols
• Collaborate with the nurse manager in the identification of problems and their resolution in order to assure the providing of safe, but cost effective patient care
• Actively participate in program related committees and/or projects
• Provide a summary report on the status of patients to the attending and medical resident on call prior to leaving each evening
• Assure proper charges have been posted and appropriate documentation provided
• Attend Program required meetings

SITES OF PRACTICE:
The advanced practice providers associated with the Hematologic Malignancy and Adult Bone Transplant Program will provide coverage and practice on the inpatient unit 9100, 9200 day hospital outpatient treatment room (North Pavilion) and home transplant patients seven days per week including holidays.

SUPERVISION:
• The Medical Director of the division will serve as the primary supervising physician for the Advanced Practice Provider staff. Evaluation of clinical and prescriptive practices will occur monthly or upon the conclusion of each clinical rotation. The PPS evaluation will occur on a yearly basis with the development of goals and objectives performed six months prior.
• The Advanced Practice Provider staff will not provide on-call services. Inpatient emergency support will be provided by the Hematology/Oncology Fellow, BMT Fellow, Designated Hospital Residents and Hematology Attending Physician. Emergency support in the outpatient clinic is provided by the ABMT Attending Physician(s) and Durham County Emergency Services.
• The Advanced Practice Providers will collaborate with the nurse manager of both the inpatient and outpatient facilities in order to optimize care and utilization of resources
