Required Qualifications: (As Evidenced by an Attached Resume)Diploma from an accredited school of Nursing or a college degree in Nursing and New York State Registered Professional Nurse. Two (2) years full time nursing experience in either a hospital or Skilled Nursing Facility.
Preferred Qualifications: Bachelor’s degree (equivalent) or higher in Nursing. Additional two (2) years of full time nursing or case management experience. Nursing experience caring for cancer patients. Experience with 9/11 Responders or the first responder population. Mental health experience, particularly in post-traumatic stress or depression. Strong working knowledge of MS Word and Excel and experience with an Electronic Medical Records system.Bilingual (Spanish/English).
Brief Description of Duties: The office based RN Medical Case Manager must possess the knowledge and experience to assess, plan and intervene as needed, to secure services and provide care coordination to patients with serious health issues. The incumbent must be able to work independently and as a member of a team. S/he must be detail oriented and have strong oral and written communication skills. S/he will be responsible for the following duties:
- Care/case management, primarily for World Trade Center Health Program (WTCHP) patients with complex aerodigestive health conditions, mental health conditions, and/or cancer.
- Educate patients re: WTC Health Program benefits and limitations.
- Perform appropriate referral and disposition planning.
- Pre-authorize covered tests, procedures and treatments.
- Provide support services to patients and families.
- Other duties as assigned
