Required Qualifications: (Evidenced by an attached Resume)
Bachelor's Degree (equivalent) or higher in Human Services or related discipline. Two (2) years of full-time relevant mental health/social work experience. Care management experience with adults with chronic physical or behavioral health conditions. Ability to transport clients to and from appointments. Knowledge of health and other community resources, rights, entitlements and advocacy.
An additional year of full-time relevant mental health/social work experience and care management experience working with adults who have chronic physical or behavioral health conditions. Fluent in Spanish and/or American Sign Language (ASL). Master’s degree in relevant human service discipline.
Brief Description of Duties:
The Care Manager provides care management services to adult persons with psychiatric disabilities and other chronic physical and behavioral health conditions who live in Suffolk County. The Care Managers support clients in the development and fulfillment of recovery and other life goals and accessing the necessary resources and supports (i.e. housing, medical and mental; health care, legal rights and entitlements, vocational and educational support, etc.) to live healthy and productive lives in their community. **Must have, keep, and maintain the appropriate valid NYS Driver's License; have a motor vehicle record which is free from major violations or a pattern of repeat violations. Applicants must submit their operator's license number on their application form for a motor vehicle check.

**This is a temporary position, which is renewed annually and dependent upon contract approval.
**This position is eligible for UUP location pay of $3,026/annually.
This is a full time appointment. FLSA Nonexempt position, eligible for the overtime provisions of the FLSA.
