Position Summary: The incumbent provides strategic direction, leadership, planning, organizational and general management for all aspects of the Case Management program which includes Case management, Transitional Care Management, Social Work, Utilization Review, Denial Management and Clinical Documentation Improvement. Incumbent is responsible for development, implementation and ongoing success of department and related organizational objectives and goals.
Required Qualifications:Ability to establish and maintain effective working relationships across the HealthSystemAbility to maintain a work pace appropriate to the workloadBachelor's degree in Nursing or Social WorkDemonstrated ability to create high-performing, interdisciplinary teamDemonstrated ability to lead change and effectively manage conflictExcellent analytic skillsExcellent working knowledge of desktop computer productivity toolsExcellent written and verbal communication skills in EnglishKnowledge of CMS, Joint Commission, CADP, and other regulatory and compliance requirementsMinimum of 3 years management experience at the Director level in a relevant health care setting in areas relevant to Case ManagementMinimum of 5 years of case management and/or social work experience in a health care settingMust be a graduate of an accredited school of Nursing or Social Work, currently licensed in California or eligible for licensureMust demonstrate extraordinary levels of customer service and transmit these behaviors to the entire care management teamMust possess the skill, knowledge and ability essential to the successful performance of assigned dutiesOutstanding interpersonal skills; ability to interact effectively at all levels of the organization from senior management to bedside staff
Preferred Qualifications: CDI knowledge and experienceKnowledge of University and medical center organizations, policies, procedures and formsKnowledge of patient safety strategiesKnowledge of quality improvement methodologiesMS, MBA or Masters in Healthcare Management or related field
