Position Summary:

The Case Manager is responsible for high level assessment and care planning for an identified group of patients which results in self-care outcomes upon discharge from the Medical Center. The Case Manager works as part of an interdepartmental team to ensure the delivery of high quality, individualized, cost-effective patient care. Salary Range:

Rate Minimum $51.46 Midpoint $51.46 Maximum $51.46

Required Qualifications:

Ability to establish and maintain effective working relationships across the Health System

Ability to facilitate and lead interdisciplinary rounds

Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload

All external candidates must have a Bachelor of Science in Nursing

Computer literacy and proficiency

Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist professional that allows for independent assessments as permitted within the scope of practice of the discipline

Excellent written and verbal communication skills in English

Familiarity of Joint Commission, CMS, CDPH and CCS requirements

Minimum 5 years of clinical experience and 2 years of previous Case Management experience or completion of the 1 year UC Irvine Medical Center Case Management Mentorship Program

Must demonstrate customer service skills appropriate to the job

Must possess complete understanding of acuity levels for specific patient population

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties

Preferred Qualifications:

BSN and/or Master's degree

Bilingual skills

CCM, ACM or Case Management credentialing

Knowledge of California Children's Services benefits authorization process (CCS)

Knowledge of University policies and procedures
