About the Unit
The Comprehensive Care Program (CCP) is located in the Section of Hospital Medicine, Department of Medicine at the University of Chicago. The program, originated as a study from the Center for Medicare and Medicaid Innovation to test whether physicians can provide better care at lower costs than the current system, allows hospital physicians to see their patients in the clinic as well as in the hospital, believing that patients will get better care, have better care at lower health care costs and be happier with their care if the same doctor is seeing them in both settings. CCP comprises physicians, study investigators and research team, and a clinical staff that includes nurses and social workers serving approximately 1,000 patients, and includes a provision for Home Care (CCP-HC) in which a physician and social worker team treat patients in their homes who cannot travel to clinic. While still under evaluation, CCP is viewed as a novel model of health care delivery and under consideration for replication in other healthcare systems.

Job Information

Job Summary:

The CCP Social Worker will spend approximately 75% of their effort focusing on clinical social work and 25% will be working on research initiatives related to C4P social needs programming.


  • Provide psychosocial support for a panel of chronically ill patients with complex social needs, including management of care transitions efforts.
  • Assess and triage the psychological needs of the patient and their family.
  • Hold family and group counseling or support sessions.
  • Contribute to the development of comprehensive service plan, assist patients in obtaining needed services, perform outreach, perform entitlement counseling, monitor and follow-up to ensure that services are appropriate and modified when necessary.
  • Work in collaboration with the interdisciplinary team and carry out daily responsibilities independently.
  • Participate in program process improvement and research activities.

  • Knowledge of principles and practices of public social welfare agencies required.
  • Knowledge of current human service issues and theories required.
  • Familiarity with evidence-based practice methods required.
  • Knowledge of federal and state laws, rules, regulations, NASW Code of Ethics, policies, and procedures as related to social work required.
  • Excellent verbal and written communication skills required.
  • Strong interpersonal skills required.
  • Ability to appreciate and respond to the needs of medically and social complex patients.
  • Ability to effectively communicate with and collaboratively engage patients, caregivers, and other service providers as part of a care team.
  • Ability to develop reports and maintain records required.
  • Ability to exercise discretion and confidentiality while handling sensitive situations required.
  • Ability to handle tasks requiring a high level of attention to detail required.

Additional Requirements

Education and Experience:
  • Advanced degree or higher in Social Work from an accredited institution required.
  • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) required.
  • Valid driver's license required.
  • 1 year of related experience required.
  • Experience working in a social service program utilizing evidence-based practice preferred.
  • Experience working with persons and families from high risk, low income, urban backgrounds preferred.
