Position Summary: The Disease Investigation Specialist (DIS)/Patient Navigator (PN) will work closely with the Wake County health department and the NC Communicable Disease Branch (CDB) on a CDC-funded project involving combined HIV and STD prevention and care for men who have sex with men (MSM) and transgender women (TGW) via network methods. The primary role of the DIS/PN is to engage MSM and TGW identified with syphilis and/or HIV at a public STD clinic and solicit information regarding their sociosexual networks. The DIS/PN will recruit and enroll index patients and individuals in their socio-sexual networks in order to conduct interviews regarding risk factors and sexual partners, and facilitate screening and linkage to STD/HIV prevention and treatment. He/she will be responsible for communicating with participants via cell phone, text, email, social media apps and all follow-up. He/she will ensure that participants receive HIV/STI services, including HIV pre-exposure prophylaxis (PrEP), and that there is communication with state DIS and HIV Bridge Counselors as needed. He/she will document connections among participants in the networks and communicate that information on a regular basis with the NC CDB.

Education Requirements: Bachelor's degree with at least 3 years of experience in community engagement.

Qualification and Experience: Prior experience with health education in HIV/STDs, and working with MSM or TGW is required. Valid driver's license required. Previous work with public health or research is preferred. Bilingual ability in English and Spanish is preferred.
