The Stroke Nurse Coordinator will work as a member of the Stroke Service team with the primary responsibility of nursing quality care for stroke patients throughout the continuum of care. They are an essential member of the stroke team and works collaboratively with other health care professionals to ensure a plan of care for each patient that is comprehensive, patient centered, and focused on returning the patient to their pre-injury state of health through quality and performance improvement initiatives and data collection.
