Position Description

Summary

The Cancer Screening Navigator is responsible for coordinating, implementing Outreach/Screening Patient Navigation for the Duke Cancer Institute through the Office of Health Equity and Disparities (OHED). Serve as a Community Navigator providing education and resources to community members and patients that seek cancer education, screening, diagnosis and treatment.

Community Screening Navigator (CSN) services may include, but not limited to, educating community members on the risks and screening guidelines of cancer, guiding patients in and through the health care system for cancer screening, helping patients to complete their insurance paperwork, assisting patients in obtaining financial help for their medical services, directing patients to their health care services for further diagnosis and referral for Treatment Navigation if diagnosed with cancer, identifying local resources and support, keeping track of and helping patients get to their appointments, talking to the patients# doctors and/or nurses, providing patients additional health care information.

Lead outreach and screening programs such as the Men's Health Initiative and the Woman's Health Awareness Day, as a link to Treatment Navigation and the health care system. Support the Director in the leadership of the DCI, Community Advisory Council. Will work directly with DCI Communications to highlight OHED's internal and external activities and maintain the OHED website.

Work Performed

Engaging the community in outreach and screening activities with follow-up documentation.
  • Provide outreach and eliminates barriers to promote cancer screening; educate clients of available community resources to improve health. Assist clients in obtaining transportation, childcare or other support when necessary.
  • Navigates patients to screening including those who are no-shows after scheduling.
  • Uses EPIC and EHR to identify and re-engage patients who were screening no-shows across several Duke clinics.
  • Coordinates outreach/screening programs including the Men's Health Initiative, the Women's Health Awareness Day, and other screening programs in the area.
  • Accompany clients to scheduled appointments and/or referral sites, as needed.
  • Maintain and update necessary forms outlining services provided, neighborhoods visited and problems encountered.
  • Coordinates volunteer outreach program and the DCI Speakers Bureau.
  • Maintains and regularly updates program database and website on outreach programs, activities and research.
Contacts key personnel at health clinics and other service/health-related organizations with access to under served populations to promote.
  • Screening Navigation services and coordinate access to cancer screenings.
  • Promotes the Cancer Screening Navigation program within the clinic and in the community, among other health care services that feed and draw from Duke Cancer Institute, to local groups, organizations and institutions, which are stakeholders in the improvement of cancer health disparities outcomes.
  • As part of the OHED team, works to educate healthcare system personnel and the community on structural, cultural and system barriers that impact access and utilization of cancer services.

Engaging in community based assessments and research activities to promote cancer prevention and screening.
  • Assists in conducting community environmental scans and health assessments of the community to identify complimentary services, programs, organizations and institutions.
  • Significantly contributes to the development of grant applications to highlight program, peer-review publications of OHED activities, and community assessments.
  • Position requires some evening and weekend hours.
  • Perform other related duties as assigned.

Education

Work requires a bachelor's degree in Public Health, Nursing, Psychology, Sociology or a related allied health field.

Experience

Two years of experience working in a related field. A master's degree in a related field may substitute for the required experience. OR AND EQUIVALENT COMBINATION OF RELEVANT EDUCATION AND/OR EXPERIENCE.

Skills
  • Strong interpersonal communication skills; is outgoing, very sensitive and aware of needs of public health clients who have limited resources to address those health needs and knowing how to effectively navigate them to services and screening.
  • Aware of key barriers to accessing care and is resourceful in addressing them.
  • Experience in conducting community assessments via surveys, focus groups, and listening sessions.
  • Strong program/project coordination skills.
  • Excellent computer skills in data entry, reporting, and the full Microsoft Office Suite.

Preferred Skills and Experience
  • It is preferred for the candidate to be a current resident of the community.
  • Familiar with organizations, institutions, health care environment, local municipal systems and services in this area.
  • Previous or recent experience working in the intervention community, preferably on health issues.

  • Prior Patient Navigation or case management experience.
  • Highly motivated and innovative, requires minimal supervision.
  • 2-5 years of experience in program coordination or management.
  • Familiarity and comfort interacting with diverse populations.
  • Professional leader with strong communication skills.
  • Driven, highly productive, innovative, and self-motivated.
  • Bilingual in English, Spanish or Mandarin a plus.
  • Valid North Carolina driver's license with a good driving record is required.
