Occupational Summary
At the Forest at Duke, a continuing care retirement community, augment the Duke Geriatric Medicine physicians' ability to provide medical services to patients;collect and document data,conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and professional consultations and provide direct patient care services.

Work Performed
  • In the clinic, assisted living and skilled nursing facility perform diagnostic and therapeutic procedures to assist physician in diagnosing medical and surgical problems and prescribing necessary treatment and services for quality patient care; record the normal and abnormal data for review by the physician.
  • Provide high quality transitional care as patient move from the clinic to the hospital to the skilled nursing facility and experience temporary and permanent transitions in their level of care needs.
  • Take patient histories, conduct physical examinations, and record the date in the patient's record;write progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care, and assume responsibility for developing therapeutic relationships with patients and their family members.
  • Order and schedule laboratory studies and diagnostic procedures; explain necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
  • Perform Medicare Wellness visits.
  • Collaborate effectively with other clinical team members including but not limited to nurses, nursing assistants, dietician, physical therapy,occupational therapy, speech language pathologists, pharmacist.
  • Discuss and document advance care planning.
  • Facilitate and participate in care plan meetings that include patients and/or their designated healthcare power of attorney or involved family members.
  • Carry out therapeutic procedures such as splint application, wound suturing and dressing changes;monitor progress of patients, report progress to supervising physician,and maintain record of each patient's progress, consulting with supervising physician when patient's progress does not meet anticipated and/or predetermined criteria.
  • Participate in quality improvement projects conducted by supervising physicians, The Forest staff and Duke geriatric medicine fellows; prepare and contribute to reports and analyses setting forth progress, adverse trends and appropriate recommendations or conclusions.
  • Review the work of lower rated employees; train new personnel.Participate in teaching students and residents in techniques and procedures which are unique to the clinic, assisted living and skilled nursing facility clinical settings.
  • Perform other related duties incidental to the work described herein.
