Radiation Oncology CNS Team· Functions as the central hub of the interdisciplinary CNS teams· Teams consists of physicians, advanced practice providers and nurse clinicians· Serves as a resource for all patient care and as gatekeeper for all patient concerns· Coordinates care with Brain Tumor Center, CNS Metastasis Program and other specialty providers throughout the Health System Clinic Preparation 15%Collaborate with administrative assistants &/or scheduling to ensure new patient records are obtained Review and summarize history, recent studies/procedures in current problem list Abstract patient records to Maestro chart Confirm films/studies/labs from referring providers have been obtained prior to clinic visit Ensure return patients have necessary tests/labs & results available for return visit Coordinate with staff to monitor clinic volumes- Review clinic templates regularly- Ensure that patients are scheduled with the appropriate providers- Solve potential scheduling problems- Collaborate with staff and providers to ensure a workable clinic flow During Clinic 50%Coordinate clinic flow - interface with front desk, clinic nurses, patient families and providers Introduce team-based approach model to patients Redo blood pressures or delegate task, follow-up results, review trends& discuss with provider Orthostatic BP or delegate task, then follow-up results & communicate with provider Present HPI, ROS and results of focused PE to MD and document in MAESTRO Provide patient teaching before and after clinic visit Function as resource for clinic nurses and clinic staff Place orders through Maestro and pend for provider signature Order CT simulations/Planning MRI Scans/Labs/Tests Coordinate & communicate with Lead Therapists to schedule radiation treatment starts Provide pre-simulation/treatment patient education Prepare the patient's after visit summary (AVS)Coordinate emergent hospital admissions-Call report to HCC, Transfer center, or admitting provider Collaborate with clinic nurses for emergency situations- Contact EMS for patient transfer and/or communicate with emergency room re: pending arrival Monitor clinic/patient points Coordinate appointments/referrals and send requests to scheduling departments - follow-up appropriately Verify and update follow-up clinic grid Maintain documentation on clinic grid sheets during clinic for follow-up purposes Medications Queue medication refills in clinic Maestro and review with provider Coordinate medication samples for patients Obtain appropriate signatures for medication samples Coordinate & communicate w/ patients & providers re: co-pay cards &patient assistance applications Follow-up with status of patient assistance program applications &communicate with patient Manage medication refill request forms received by fax Refill prescriptions as provider directs Complete medication pre-certifications or prior authorizations (atleast 30 minutes each)Follow-up communication with patient and providers re: approvals and/or denials If prior authorization denied, discuss plan of care w/ provider & relay changes to patient Daily Triage Calls 5%Monitor Maestro in-basket for patient call notifications Respond immediately to semi-urgent and urgent message received by pager Return patient calls, collaborate with staff & providers and communicate plan of care to patient Document patient communication in telephone encounters Follow-up 20%Follow-up on labs/imaging within 24 hours of clinic visit Annotate normal labs/procedures and communicate with patients via MyChart, letter &/or phone Document patient communication in Maestro chart Follow up/ensure - labs/studies/tests/simulations are scheduled and completed Evaluate trends for abnormal results & review/discuss abnormal results with provider Call patients with explanation of abnormal results and plan of care Educate patients-in clinic and after clinic via phone-(labs/studies/simulation/treatment/meds/diseases)Provide printed patient education material - continue to develop appropriate materials Schedule/coordinate lab/studies/procedures at outside facilities(calls/faxes/e-mail)Coordinate with financial care counselor re: insurance approvals If insurance coverage denied, discuss plan of care with provider and patient Manage records from outside facilities- Request external records (labs/studies/procedures/notes/imaging films)- Follow-up receipt of records(labs/studies/procedures/notes/imaging films)- Review received records/films with ordering provider- Communicate the results of external labs/studies/procedures to patient- Abstract/document external records to Maestro chart- Carry out any further orders related to plan of care on external results are received & reviewed Obtaining timely appointments with specialists/coordinate w/ pt Review all follow-up grid sheets after clinic-to ensure items were completed Follow-up on dexamethasone taper and modify schedule, as appropriate, depending on response Formal Paperwork Coordinate Rehabilitation referrals (cardiac, vascular, pulmonary, physical therapy)Obtain signed orders and fax the following:- Home Health referrals- Home Hospice Complete disability applications Complete paperwork for indigent prescription assistance Obtain appropriate provider signatures on patient assistance program applications Research 10%Screen all patients for inclusion into current clinical research protocols Coordinate referrals & communicate with research coordinators Educate staff nurses on protocols and regulatory compliance Educate patients re: available research protocols/studies
