Job Description

The Children & Family Services Social Worker/Support Coordinator duties are performed primarily in the home and community for children and their families, including case management, crisis intervention, coordination of multiple services and supports, and therapeutic interventions. This program provides an integrated and comprehensive home-based child parenting and family support program to families with a child who has a developmental disability and/or Severe Emotional Disturbance, and who requires assistance and support to address issues identified by the family and their circle of supports, which are affecting their capacity to provide healthy parenting for their child. The aim of the service is to identify needs and support families by developing a plan in conjunction with the family that takes into account family's strengths and willingness to change. The family plan also will make use of formal and informal support services with the objective of enhancing family functioning/resilience.

Much of the work of this position is performed independently and requires considerable knowledge of family and child centered services and developmental disabilities as well as knowledge of local, state, and federal regulatory standards. The employee receives general supervision and works within established protocols and procedures in the form of regulations, rules, policies, and oral instructions.

Qualified candidates must possess, at minimum, a bachelor's degree with major in social work and a Bachelor's Social Worker License is required
