Clinical Role:
The cardiothoracic Nurse Practitioner/Physician Assistant will provide medical and surgical care for patients requiring cardiothoracic surgery. The cardiothoracic Nurse Practitioner/Physician Assistant will perform these services under the direct supervision of the faculty physician(s). The cardiothoracic Nurse Practitioner/Physician Assistant will provide perioperative care in various medical center settings. The cardiothoracic Nurse Practitioner/Physician Assistant will be involved with the care of the patient in any role which the supervisory physician deems proper and consistent with applicable law, medical center regulations, delineation of privileges, and the training, knowledge, skills and experience of the cardiothoracic Nurse Practitioner/Physician Assistant.

Scope of Practice:
The cardiothoracic Nurse Practitioner/Physician Assistant provides care to patients of all ages (newborn to elderly adult). Below are listed basic functions performed by the cardiothoracic Nurse Practitioner/Physician Assistant. This list is not all inclusive. All functions are at the discretion of the supervisory faculty physician.

• History/ physical examinations, and consultations
• Admission and perioperative orders including consultations
• Data acquisition/analysis and database maintenance
• Coordinates/communicates patient care with nursing staff
• Participates in patient/family education
• Communicates with referring/consulting house staff and physicians as well as outside referring physicians.
• Obtains data for surgical research
• Initiates/participates in resuscitation of patients ( by ACLS standards )
• Participates in medical chart maintenance
• Participates in all aspects of follow-up care for discharged patients through clinic visits and phone calls.
• Evaluates pre and postoperative patients in the clinic setting
• Initiates orders for medications
• Preoperative case planning for Thoracic Endovascular Aneurysm Repairs
• Participate in clinical trial enrollment
• Order and schedule laboratory studies and diagnostic procedures; explain the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
• Perform procedures such as simple wound debridement, thoracentesis, suturing and dressing applications.
• Write/dictate progress/interval notes and narrative discharge summaries; prepare patient workups and summaries noting pertinent positive and negative findings.

Prescriptive Privileges:
The cardiothoracic Nurse Practitioner/Physician Assistant will prescribe medications for inpatients and outpatients.
This includes both narcotic and non-narcotic agents used for the treatment of cardiothoracic patients. These privileges will be in compliance with the North Carolina Board of Nursing regulations, federal laws, and Duke Medical Staff regulations governing Nurse Practitioner/Physician Assistants.

Periodic Review Policy by Supervising Physician for Drug prescribing:
Periodic review of drug prescribing by the cardiothoracic nurse practitioner/physician assistant will be performed by the faculty surgeon on a regular basis this will consist of regular review of drugs prescribed to inpatients by medical record review and to outpatients by clinical record review. The Performance Partnership System of evaluation will provide the Faculty physician and cardiothoracic Nurse Practitioner/Physician Assistant a format to assure conformity of drug prescribing.

Access to Supervising Faculty Physician:
The cardiothoracic Nurse Practitioner/Physician Assistant will have access to the supervising faculty physician on a twenty-four hour basis, either by direct or electronic contact (telephone, beeper, etc.) There will be a back-up faculty physician for emergencies, vacations, etc.

Process of Evaluation:
Initially the cardiothoracic Nurse Practitioner/Physician Assistant will meet with the supervising physician on a monthly basis for review of training/performance progress during the initial 90 day probationary period. This will provide a basis for the cardiothoracic Nurse Practitioner/Physician Assistant and the supervising physician to establish an ongoing job performance/progress system by utilizing the annual Duke PEP. The faculty physician may designate a senior cardiothoracic Nurse Practitioner/Physician Assistant to administer the Performance Evaluation and Planning process and provide verbal and written reports on training/performance progress.

Qualifications:
Work requires a bachelor's degree in Health Science and a Physician Assistant Certificate of Registration with the North Carolina Medical Board or be certification eligible, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants -OR-completion of accredited Nurse Practitioner program. State of North Carolina licensure is also required. The successful applicant must either have experience in endovascular vein harvesting or comparable surgical training and assessment that indicates an ability to do so. Experience in critical care and the management of cardiovascular patients is preferred.
