We are seeking Field Nurse Case Manager to provide services in Bronx and Westchester areas.
Assesses member needs and identifies solutions that promote high quality and cost effective health care services. Manages requests for services providers, members, care management team and renders clinical determinations in accordance with client's policies as well as applicable state and federal regulations. Delivers timely notification detailing clinical decisions. Coordinates with the team, subject matter experts, physicians, member representatives, and discharge planners to ensure care is appropriate, timely and cost effective. Works under general supervision.
Licensure: Current license to practice as a Registered Professional Nurse in New York State required. Certified Case Manager preferred.
Education: Bachelor's degree or Master's degree in nursing preferred.
Experience: MUST have MINIMUM 1+ years of experience either in Home Care or in Tele/Med-Surg. or Skilled nursing. Driver's License and proof of Vehicle ownership. 3 week orientation mandatory client will provide handheld computer for Case Notes. Contract personnel will be reimbursed for miles. Knowledge of Medicaid and/or Medicare regulations required. Knowledge of Milliman criteria (MCG) preferred.
