Required Qualifications: (As Evidenced by an Attached Resume)
Associate's Degree. In lieu of a degree, two years of full time experience in a health education/advocacy or case management or patient care role. Four years of full time experience working in a healthcare setting with direct patient/client contact and a role dealing with patients/clients issues. Knowledge of medical terminology. Proficiency with Microsoft Word and Excel.

Preferred Qualifications:
Additional years of experience. Training in customer service. Skilled at effectively dealing with irate customers and/or hard to motivate patients. Experienced at navigating complex health systems and assisting clients/patients to achieve treatment goals. Familiarity with tablets. Familiarity with research protocols, informed consent and collecting information. Familiarity with HIPAA rules. Experience with EMR and other patient-based databases. Familiarity with the World Trade Center Health Program and its benefits.

Brief Description of Duties:
The successful candidate must possess excellent verbal communication skills and be able to effectively communicate complexities in a simplified manner. Strong organizational skills and attention to detail are also required. Must be able to work independently and as part of a team. S/he will perform the following:

  • Act in coordination with other clinical staff to ensure each patient at our clinic has a positive experience during their monitoring visit.
  • Ensure patients individual concerns have been attended to in a caring and efficient manner.
  • Ensure that there is an open line of communication between the patient and the clinic.
  • Educate members either before or at their monitoring visits on WTC Health Program benefits, changes in the Program, local events, activities and research opportunities.
  • Assist members by answering questions, help to resolve issues and help members navigate the Program.
  • Meet face-to-face with new members and members who have not returned for many years and review the Program and the consent forms.
  • Conduct assessments with patients to measure physical functioning and collect information for research and case management purposes. This task involves utilizing a PC application and tablet to enter data.
  • Respond to patient calls.
  • Assist the case management team by eliciting information from the patient. Find the best person to deal with the patient's issue and/or direct the call to the appropriate case manager.
  • Assist with retention tasks that allow for the evaluation of retention efforts, such as member surveys.
  • Log all interactions with members and track problems and complaints.
  • Ensure timely follow-up.
  • Other duties as assigned.

Special Notes: This is a full-time position, 37.5 hour per week, Monday through Friday, 8 am - 4 pm. Occasional Saturdays will be required. This position is at our Commack clinic. Travel between the Commack and Mineola clinic will be required. FLSA Nonexempt position, eligible for the overtime provisions of the FLSA
