Required Qualifications: (As Evidenced by an Attached Resume)

Diploma from an accredited school of Nursing or a college degree in Nursing and New York State Registered Professional Nurse. Two (2) years of full time case management experience. Two (2) years full time nursing experience in either a hospital, ambulatory or medical office setting. Must have strong working knowledge of MS Word and Excel and experience with an Electronic Medical Records system.

Preferred Qualifications:

Bachelor’s in Nursing. Additional two (2) years of full time nursing experience. Nursing experience caring for cancer patients. Additional two years of full time case management experience. Knowledge of MS Access. Experience with 9/11 Responders or the first responder population. Mental health experience, particularly in post-traumatic stress or depression. Bilingual (Spanish/English). Experience with Workers’ Compensation.

Brief Description of Duties:

The Medical Case Manager must possess the knowledge and experience to assess, plan and intervene as needed, to promote health, secure services and help patients cope with serious health issues. The incumbent must be able to work independently and as a member of a team. S/he must be detail oriented and have strong oral and written communication skills. S/he will be responsible for the following duties:

  • Care/case management, primarily for World Trade Center Health Program (WTCHP) patients with complex aerodigestive health conditions, mental health conditions, and/or cancer.
  • Educate patients re: WTC Health Program benefits and limitations.
  • Perform appropriate referral and disposition planning.
  • Pre-authorize covered tests, procedures and treatments.
  • Provide support services to patients and families.
  • Assist in the clinic as needed.
  • Other duties as assigned.
Special Notes: Occasional Saturdays will be required. Occasional travel between Commack and Mineola clinics will be required.

FLSA Nonexempt position, eligible for the overtime provisions of the FLSA.
