NURSE PRACTITIONER - MEDICINE NEPHROLOGY

The APP provides medical care to adult in-patient in the Duke University Health System (DUHS), in collaboration with physician members of the division's Clinical Practice Group. The APP will work with the faculty and current APP in order to evaluate and provide treatment to patients admitted to DUHS and in need for consultative nephrology service, in accordance with established scope of practice as specified in North Carolina Board of Medicine Medical Practice Act NCGS Chapter 90 for Medicine and Allied Occupations.

The Division of Nephrology provides care to a large number of hospitalized patients who require consultation for acute kidney injury, chronic kidney disease, end-stage kidney disease requiring dialysis, and also fluid and electrolyte disorders. New consults are generally seen by a physician and APP or fellow jointly.

This position will involve care of patients on the Acute Nephrology Service and Maintenance Dialysis Service. It is anticipated that this position will involve primarily inpatient care, but that in the future there may also be an outpatient component.

WORK HOURS
There will be occasional weekend duty, but no night call. Specific daily activities include:

Overall Responsibilities
# Pre-round on established patients assigned to the APP.
# Round jointly with the dedicated attending on established patients assigned to the APP.
# See new consults for acute kidney injury, chronic kidney disease, end-stage kidney disease.
# Record history, past medical history, social history, review of systems, exam, assessment and plan in Maestro.
# Establish and follow plan of care in conjunction with MD
# Provide patient and family education in the context of daily rounds
# Order appropriate labs, tests and procedures and follow up with patient as needed # Coordinate care and refer to other medical professionals as needed
# Review lab results in Maestro, communicate and interpret results
# Provide orders for dialytic therapies for patients with end-stage renal disease and patients with acute kidney injury jointly with the attending.
