Discharge Planning Director
Carolina Health Care Center of Cumberland has an immediate opening for a Director of Discharge Planning. The Discharge Planning Director directs and supervises the discharge planning department within the framework of MFA's Standards for best practices and in accordance with state and federal regulations. Responsibilities include coordinating all discharge planning initiatives upon admission and works with the patient, family and interdisciplinary team for transitioning home or to an optimal care environment.
The facility provides a high volume of short term nursing and rehabilitation services and our candidate will provide discharge planning as well as comprehensive medically related social services to our patients. Responsibilities include MDS, Care Plans, Assessments and timely documentation operating within the framework for our company's standards for best practices in accordance with all state and federal regulations.
Qualified candidates will have a 4 year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice or other human services related degree.
