Associate's degree. In lieu of degree, a combination of directly related education and/or experience totaling two years may be considered. Experience in data collection, data entry and data tracking. Must be computer literate.

Preferred Qualifications:

Bachelor's degree in health related field. Proficiency with MS Word and Excel. Familiarity with Power Chart (EMR). Previous experience working in a medical outpatient clinic.

Brief Description of Duties:

The Patient Navigator will serve as a point of contact and will be a resource for physicians, patients, and caregivers by providing assistance with accessing clinical and supportive care services offered after diagnosis of positive Hepatitis C Antibody results. This position will also assist with expediting patient appointments, including those made with labs, diagnostic areas, and specialty physicians. Incumbent will possess excellent communication and follow-through skills.

  • Assist patients in understanding their diagnosis, treatment options, and the resources available.
  • Provide linkage to care with either their primary care provider, their primary specialist who treats viral hepatitis or refer to providers at Stony Brook Medicine.
  • Follow patients through from diagnosis to first outpatient appointment.
  • Input information into database created by the data analyst and project manager.
  • Ensure timely acquisition of laboratory data and communication to all relevant parties.
  • Work with Data Analyst and Project Coordinator to collect data, track outcomes, and support strategic planning processes. Prepare and present reports of weekly activity to the project manager and principal investigator.
  • Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, diagnostic staff, nurses, laboratory staff and primary investigators.
  • Other duties as assigned.
  • Special Notes: FLSA Nonexempt position, eligible for the overtime provisions of the FLSA.
