At Stony Brook Medicine the Instructional Support Assistant in Care Management will be vital in supporting Case Managers and Social Workers by fulfilling all request for transportation, DME services, and obtaining pertinent prior authorizations from Medicaid to facilitate post-acute placement and discharge. The staff assistant will perform clerical and office support work; such work may consist of, but is not limited to answering telephones, taking messages, paging staff, answering insurance inquiries, and assist in picking up, filing, delivering and distributing mail, faxes or other forms of correspondence and communication.
Duties of a Instructional Support Assistant in Care Management may include the following but are not limited to:
Coordinate transportation for ambulances, ambulette, and taxi’s for all inpatient hospital discharges and Emergency Room/CPEP discharges.
Assist with obtaining DME services for all inpatient/psychiatry hospital discharges including filing and distributing necessary paperwork and forms.
Assist with obtaining prior authorizations from Insurers for post-acute services including, Skilled Nursing Facilities, Home Care, Hospice, and DME. This includes coordination of beneficiaries who are in need of an ambulance, Ambulette, or taxi for discharge.
Answering phone calls and triaging the caller to the appropriate party (e.g.: Unit case manager or social worker, executive leadership, etc.) and providing excellent customer support for patients and family members.
Act as a liaison between the Case Managers and insurance companies by answering insurance inquiries by phone call or fax correspondence for additional utilization reviews, requested discharge dates, authorization for post level of care etc. Communicate unique and complicated cases to denials and appeals unit.
Track and trend transportations arranged quarterly that is evaluated by type of transportation and payment method (private pay, authorization, and department vouchers)
Provide application support to Case Managers and Social Workers by Troubleshooting issue users in both Cerner and Navihealth.
Required Qualifications: Minimum level of formal education is a Bachelor’s degree in a Health Care related field with at least one year experience working in a hospital setting. At least one year of working knowledge of Insurance, Electronic Medical Record System (Cerner Power Chart).Understanding of post-acute placement services. Understanding of transportation benefits for hospital discharge. Application such as Microsoft Excel, Word, and PowerPoint, and experience facilitating and coordinating transportation.
