Bachelor's Degree in a health-related field. Minimum three (3) years of full time, directly related management experience. Five (5) years of case management experience. Familiarity with Cancer Case Management. Proficiency in MS Word and Excel.

Preferred Qualifications:

Master's degree in a health-related field. Two (2) years of Case Management supervisory experience. Two (2) years of Cancer Case Management experience. Working knowledge of NCCN guidelines. Knowledge of WTC Health Program policies and procedures. Knowledge of Workers' Compensation regulations.

Brief Description of Duties:

The Case Management Manager is responsible for monitoring and supporting case management operations and staff, ensuring standards are maintained at two locations while following World Trade Center Health Program (WTC HP) quality standards. The successful candidate must possess excellent communication skills. S/he should be self-directed, organized and have the ability to manage priorities in a timely manner.

Duties will include but are not limited to:

  • Monitoring case management and support staff activity to ensure that the case load is distributed appropriately, the services delivered are in keeping with World Trade Center Health Program Clinical Center of Excellence quality standards and that each site operates efficiently. Reassign the workload as needed to maximize productivity.
  • Monitor the delivery of case management services to patients at each location ensuring compliance with all aspects of case management care in accordance with Stony Brook and WTC HP policies and procedures.
  • Assist with coordination of certification process as needed
  • Provide oversight to the cancer screening program
  • Identify and supervise ongoing staff training to ensure proficiency and technical competence among employees.
  • Ensure that case managers work to empower patients and family members to be active participants in their care and assist them in navigating the healthcare system.
  • Provide back up to case management staff as needed.
  • Ensure standardized protocols and quality initiatives are adhered to so as to improve patient outcomes.
  • Ensure processes are in place to standardize case management services at both locations.
  • Ensure compliance with referral, certification and authorization policies, procedures and processes.
  • Assist with the creation and implementation of policies and procedures regarding case management.
  • Maintain compliance with federal and state regulations and contractual agreements.
  • Participate in Clinician and Administrative meetings and other meetings as necessary.
  • Manage case management operations in an independent manner with minimal day to day supervision/direction but work closely with the Clinical Administrative Manager and Deputy Director over concerns with meeting goals, operational obstacles, or compliance issues.
  • Work closely with other management staff to ensure excellent integrative approaches to patient issues.
  • Develop and maintain effective ongoing communication and effective working relationships with physicians, support team, supervisors and patients.
  • Facilitate on-going communication between Case Management staff, patients, providers, and other involved entities.
  • Conduct case reviews for progress, quality and accuracy.
  • Compile and review multiple reports for tracking purposes to identify case management trends and assist in forecasting.
  • Interview, recommend for hire, ensure the training and on-boarding of new clinic staff while managing performance, feedback and placement of current team.
  • Oversee and provide support, guidance and coaching to case management team and team leaders.
  • Monitor and approve submission of time sheets and vacation and sick time requests.
  • Complete performance evaluations and disciplinary paperwork as needed.
  • Other duties as assigned.

Special Notes: This is a full-time position, 37.5 hours per week, Monday through Friday, 8:30 am - 4:30 pm. Occasional Saturdays will be required. We are located in Commack. Travel between the Commack and Mineola clinics will be required. FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain the FLSA exemption.
