The University of Virginia's Department of Student Health, Counseling and Psychological Services (CAPS) is seeking to hire a full-time Care Manager position. The Care Manager collaborates with CAPS and other Student Health staff in developing, maintaining, and monitoring a clinical case management system aimed at providing continuity of care and follow up for UVa students. The Care Manager assists at risk or high risk students with providing rapid triage, close clinical monitoring, evaluating the student's developmental and psychosocial needs, to effectively and efficiently determine the student's best fit for available resources, and will ensure linkage/connection to appropriate treatment. The Care Manager actively assists students with navigating insurance, seeking referrals to community resources, and gaining access to care (including coordination of hospitalization and discharge planning). The Care Manager maintains liaisonships with University stakeholders as well as the UVa Health Care System and other important community constituents. This position further assists in the development and maintenance of a database of referral resources and community services in the surrounding community. The CAPS Care Manager provides day and after hours on-call crisis services as an active CAPS team member, consultation to the UVa community, triage, some time-limited therapy and/or group therapy, and relevant outreach to UVa and/or community partners. Additionally, this position provides further integral support, training, and possible supervision, to trainees in an APA-accredited doctoral internship program.
