Position Summary: WakeBrook Primary Care Services, part of the Department of Psychiatry at the University of North Carolina at Chapel Hill, seeks to hire a full-time (40 hours per week) Clinical Instructor faculty member. The position will work closely with primary care team clinicians, nursing staff, peer support specialist and health educator to provide coordinated approach to primary health care for patients seen at the Primary Care practice at the WakeBrook Crisis Facility. Principal duties will include: * Assure that patients are appropriately connected to behavioral health services. * Improve communication between behavioral health teams and primary care team * Assure that patients follow up with specialty referrals and testing * Improve communication between specialty providers and primary care team * Work with patients to increase understanding of their chronic physical health illnesses and behavioral health issues * Develop and implement interventions to improve engagement and increase attendance at primary care, behavioral health and specialist appointments; * Develop and implement interventions to assure appropriate use of the emergency department. * Assistance with accessing appropriate resources for management of physical health illnesses; * Assistance with accessing appropriate resources for management of behavioral health issues; * Assistance with accessing appropriate and cost-efficient services and/or supports in the community; * Assistance addressing barriers to healthcare including but not limited to financial, transportation, appropriate housing. Coordination with Charity Care and Project Access to assist uninsured participants. * Collect and report data on effectiveness of the work The office is located in Raleigh, NC.

Education Requirements: Applicants for the Clinical Instructor position must have a Master's degree in a relevant human services field preferred.

Qualification and Experience: Direct work experience with persons with severe and persistent mental illnesses. Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment and services are essential. Knowledge of mental illness and mental health and social service systems, ability to navigate complex systems and to participate as member of multidisciplinary team required.
