Cystic Fibrosis Clinic seeks a Licensed Clinical Social Worker (LCSW) to work as part of a multidisciplinary primary care team performing behavioral health assessments and counseling services and care coordination. The position will include the delivery of behavioral health counseling visits at the Kentucky Clinic serving cystic fibrosis patients. In addition to behavioral health counseling, this position will perform integrated behavioral health care case management services via coordinated care with primary care providers and clinic staff.

Physical Requirements

Position may involve long periods of sitting and will involve completion of behavioral health counseling notes on an electronic health record using computer resources and typing. Position may also involve navigating the length of the clinic on hard surfaces.


Monday-Friday, 8:00am-5:00pm (40 hrs/week). No holiday or weekend shifts.
