Position Summary

At Stony Brook Medicine, our CPEP Follow Up Social Worker, is responsible to coordinate all patients that have been evaluated and discharged from CPEP with outside services and outpatient providers to ensure continuity of care. They run daily reports on recently discharged patients ensuring all who need follow-up phone calls have one ordered. Also they ensure that all discharge phone calls are completed on patients that are deemed “at risk” are completed. Coordinating with the Suffolk County Mobile Crisis team, the social worker maintains records to ensure that patients are compliant with follow up appointments,as well as, ensuring all clinical information is sent to their current outpatient provider for continuity of care.

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Duties of a CPEP Follow Up Social Worker may include the following but are not limited to:

  • Completion of Community linkages and follow up calls for all patients evaluated in CPEP. Including following up with providers to ensure all information has been received, utilizing the Follow-up list to accurately document in the EMR.
  • Completion of follow up calls and community linkages for high risk In-patient adult psychiatric discharges
  • Documentation of all follow up activity in medical record. Track information as data.
  • Consultation with CPEP team and representatives of Community Agencies.
  • Coordination with Suffolk County Mobile Crisis Team, and other oversight agencies in order to improve patient’s compliance with follow up appointments. Including follow up letters for those who have missed appointments.
  • CPEP/ED coverage as needed for Acute Psych and Medical/Psych Social Work cases
  • Run daily reports for follow up phone calls and failed fax reports. Ensure all discharge phone calls are ordered and made on all patient’s that are deemed at risk within a 72 hour period and documented in the computer system
  • Weekly Supervision.
  • Qualifications:

    Required Qualifications: Master’s degree in Social Work. New York State Licensure within one year of hire. Knowledge of OMH, OPWDD, and Substance services that are available in the county. Experience working with the OMH, OPWDD, and Substance population as well as their families.
