The primary purpose of this position is to coordinate and support care team members through an interdisciplinary medical home health care system by assisting providers and patients with evaluation, health screenings, treatment plans, referrals, education, emotional support, preventative care, health promotion and tracking interventions and outcomes. The Care Team Coordinator also functions as a registered nurse and as an advocate for patients with internal and external service providers to improve patient outcomes. Support is guided by principles based on the needs and values of the patient and are accomplished in collaboration with all service providers in a manner that supports appropriate patient centered care. Functions as the liaison between patient referrals and providers to include; arranging appointments, reviewing alternate resources and facilitating connections to care, follow-up, establishing care plans, education and discharge disposition. The Care Team Coordinator works within the context of a primary care medical home model to promote timely access, continuity of care and the enhancement of patient education and well-being. Individual will work with information technology to advance the electronic medical record in regards to disease management/tracking features. This position will typically work 40 hours per/week; M-F 9:00am-6:00pm. Sundays 2:00pm-8:00pm on rotating basis during fall and spring semesters. Position will typically work 40 hours per week, however based on duties of position, individual may be called to assist with campus and departmental crisis during evening and weekends and during peak periods of high patient volume. Position is considered essential personnel during emergency and weather related incidents.
Master's degree and 2 years experience, or Bachelor's degree in nursing and 4 years experience or diploma from accredited school of nursing and 5 years experience. Licensure as a registered nurse by the South Carolina Board of Nursing.
Excellent nursing skills and advanced patient assessment techniques and abilities. Three years experience in a nursing, care manager or case manager type position. Experience with electronic medical records that incorporates disease management and/or tracking. Experience in analyzing information and using logic to address patient/client-related needs, expectations, issues and problems. Ability to work cooperatively, effectively and in collaboration with diverse groups of people towards a common goal. Is passionate for a holistic patient care delivery model to include primary care, preventative care, mental health, health education and promotion and risk reduction. Experience in public health highly desirable.
