Position Summary: The Physician Clinical Faculty provides comprehensive preventive and primary care services consistent with PCMH principles, as part of the clinical provider team, to patients of Bluegrass Community Health Center. The incumbent is accountable for the clinical leadership of their healthcare team. The Physician Clinical Faculty must assess, diagnose, and treat a large variety of acute and chronic conditions and promote wellness within the scope of general practice. The Physician's health care services must be provided with a Patient Centered Medical Home approach, including managing their panel of patients, and with consideration for the cultural and linguistic needs of the patient. The Physician will ensure that practice is HIPAA compliant. The Physician will be an active participant in health center quality activities and will work to promote positive health outcomes for health center patients.
Minimum Educational Qualifications: A degree of Doctor of Medicine or Osteopathy is required.
Minimum Licensure/Certification Qualifications: Must be licensed to practice medicine in the state of Kentucky, or eligible for licensure, as required by KRS 311.560, and must hold a valid DEA certificate or obtain one by the first day of employment. These credentials and their associated fees are solely the responsibility of the physician.
The candidate is required to maintain continuing medical education credits in quantity sufficient to satisfy requirements for re-licensure.
Minimum Experience Qualifications: Completion of an approved residency program is required.
